Your hospital team refers you to Post Acute Specialists.
After you leave the hospital, we assign a dedicated physician who specializes in post-hospital recovery.
2. Introduction
We reach out and schedule a video visit within 72 hours.
Whether you’re going home, to a skilled nursing facility, or assisted living, our doctors meet with you by video. No travel required.
3. Consult
A Post Acute Specialists doctor reviews your discharge plan, medications, and next steps.
Following your discharge, an Inspire PAS physician will assess your care plan and medications to ensure a safe and smooth transition home.
4. Results
We coordinate with your primary care and community specialists.
We coordinate care across your medical team—including your primary care doctor and local specialists—for better outcomes.
